Request Medical Records

At Specialty Pain Management, it is important to us that you have access to your medical records. That is why we make it easy for you to request access or make changes to your records.

To request your records, call our Medical Records Department at 480-496-2699 and we will mail or fax the Release of Information Authorization to you. Once completed, you can return the Authorization to an SPM office or you can fax it to our Medical Records Department directly at 1-877-422-3184.

The turnaround time to receive your Medical Records is typically 14-21 business days depending upon your request; however, we strive to complete your request as soon as possible.

If you would like to make a correction to your Medical Records, call the Medical Records Department and ask for an Amendment Form. This form can be faxed, mailed or picked up from any SPM office. Once the completed form is returned to the Medical Records Department, SPM will discuss any requested changes with your medical provider(s). When a decision is made whether or not to make an amendment to a record, we will contact you.
